Improve Harvesting Reports #116

Increase usability of the harvesting reports to help the harvester user understand failure in the process. The harvest report needs to distinguish:

collecting source documents
any processing that happens
failures in publishing to the supported output brokers

Regarding publication failures, the different output brokers may have different responses and this needs to be accounted for in the report.
