Subset UGRID format with support for coordinate re-indexing and large data files. A low memory overhead is desirable so generators and have file IO is preferable for the first-cut.

  source indexing.

Driver support added for UGRID. This does not include element-element connectivity subsetting/re-ranking. This is part of #464.
