Unfinished items for version 2.3:

- Add ALSA direct-FM interface support.

- When only one OPL2 is active, it needs to be output to the center channel.

General to do items:

- Add new Protracker loaders:
  - MTR: Arkham Master Tracker [riven-mage]

- New players:
  - Palladix file format, used in LOGICAL game [dynamite]
  - Herad System (HSQ), used in Dune, Megarace, KGB games

- Add possibility to mute/solo AdLib channels.

- Isolate Ken Silverman's synth (no global variables) so that multiple
  instances can be created.  Surround mode won't work with Ken's synth
  until this is done.
