Hello, the "name" field is not included in the "new risk" form.

I noticed the same issue. The field exists in the table but is not in the form. I was able to add the field to the form.

Solution:
In Power App: I went to solutions --> select the solution --> Projects --> Edit in Menu tab, Navigated to risk form page, then clicked on the edit pencil icon. Searched for "Name" in the column fields and dragged and dropped where i wanted it.

In my experience, I suspect the [Name] column to not be included is that it appears to have a Key set against it (meaning only a single instance of the risk name). There is a [Subject] field provided as single line text that doesn't have the key and thus allows for the same risk title to be used across other projects. You need to be aware of the Key against the [Name] field.
