Actually, values displayed in QGIS forms are values stored as Alias :

Translations should read the field names and define the alias from it (in its translated version).

try to add a line in additional translations like here
https://github.com/qwat/QGEP-customizer/blob/master/i18n/fr.yaml#L19

if the layer is not listed yet, it has to be added.
