Make link between episode_parent_id and episode_id #95

OHDSI has a bug in their documentation: episode_parent_id is not a foreignkey, while it should be
Issue has been made at the ohdsi platform: OHDSI/CommonDataModel#707
In the mean time it should be solved by hard coding it into riab (like it was done for note_nlp: https://github.com/RADar-AZDelta/Rabbit-in-a-Blender/blob/main/src/riab/etl/etl_base.py#L107)
