or_other not appearing in field-lists

I'd like to avoid this issue by making "specify other" questions appear after the field-list rather than in it.
However, or_other questions aren't implemented in a way that makes this easy to do (I would rather they were processed entirely in the the xls2json code), so I'm not going to get to it right away.
