Who can post forum topics and responses to these topics?

When discussing the forums with the NMLA testers, it became clear that defining who could post new forum topics is important. We don't want to overcrowd the site with hundreds of forums, yet users should be allowed to create groups. It can also be a pain to have to wait for admin approval.

Which then brings up the topic of moderation - how do we monitor the forums for abuse?
