Create a page to demo various styles

We should create a page that is only accessible for test and development environments. It should display sample forms, inputs, page styles etc. This will help us to ensure that we can view these styles in one location in case any style changes have an adverse effect.

Eventually we could plug in something like wraith to enable us to test this in our integration tests.
