SUBJECT: GDPR Right of access and data portability request

Dear Sir, Madam,

I would like to exercise my GDPR right to access information. I am a data subject in the context of your service/website ..., registered under the email address/account ...

Please provide me with:

  • confirmation that you are processing my data and that you assume either the GPDR data controller or data processor role for this data
  • the lawful basis that you are using for processing my data
  • access to the personal data you hold of me
  • supplementary information on how my data was processed and stored.

Please provide this data in a structured, commonly used and machine readable form (e.g. CSV files).
