Change name of "Informed Consent" resource #39

Now we use this terminology:

"Consent Document" = the document to show to the user as part of the onboarding process
"Informed Consent" = the version of the consent document shown to, and signed by the user, containing the exact wording and translation as shown.

This should be relflected in the portal. For example, this dropdown should be changes to "Consent Document":
