• I think you need to say something about what happens to the data entered. There may be situations where the costs are confidential for example, so people wouldn't want to use it unless it said the data would not be stored or used in any way.

• In step 4 I think it should say 'The tab shows you the first and last few rows.'

• For the table I prefer to see it the other way around eg. different treatments down the rows and costs and QALYs in separate columns, which may be because this is the way it is most often presented. It does mean that you can fit multiple comparators down the rows, which may not fit across if there were lots.
