In paragraph starting "Depending on the connectors limitations..." the refresh tokens and group membership links are broken.

@sagikazarmark, both these links are incorrectly linked, but looking at it closer they are both pointing to the same place -- when we change the link, should they still both link to: https://dexidp.io/docs/custom-scopes-claims-clients/#scopes?

I've opened PR #37 for this to at least stops the links from 404ing.
We can have further discussion about where we would want these to link more permanently.
