I think I solved this in my rework: https://github.com/madig/HTLetterspacer/blob/85aaa90/src/htletterspacer/__main__.py#L139-L151

The idea is to keep a mapping of glyphs to composites that use that glyph as a component and then move these components in the opposite direction (measured to the origin) of the left margin spacing.
