Deprecated Notice and Error afterwards #124

i was testing this function and its showing me a deprecated notice

Notice: The give_after_donation_levels action is deprecated since GiveWP version 2.10! Use give_fields_after_donation_levels instead.

Once I use 'give_fields_after_donation_levels' instead, the whole form breaks away like this

I just want to add a field in the form.
