Introduce proper citations

Currently, we point to other documents by merely mentioning their titles and URLs in running text. We shall turn to using proper references, using citations (best in author–year format) and a list of references. In doing so, we shall also rephrase text around citations where appropriate; for example, we shall remove explicit mentioning of book titles and move pointers to sectional units into citations.
