Bunch of error 0x402 which is fs error Path already exists. Seems to be similar to this issue #403. I have no idea why you would be getting this error. I'm tempted to say it may be a problem with your sd card, given how popular this controller is and how the issue isn't being widely reported.

Did you delete the sdmc:/config/MissionControl/controllers directory?
