Move calendar color from CSS into specification

The calendar color is generated as CSS client side. This means that CSS improbvements and changes do not go into used calendars.

These classes:

.CALENDAR-INDEX-0, .CALENDAR-INDEX-0 .dhx_body, .CALENDAR-INDEX-0 .dhx_title
