Differentiate VN and WHO case report forms #38

Currently we are using the VN case report form. At some point we should move that to a hiv-ig-vn IG specific for Vietnam and have the questionnaire in this IG implementing the WHO model Case Report Form:

The data fields in here should map easily to the WHO HIV-DAK data dictionary. If not, we should flag for Jenny T.
