Update comment logic is broken #224

Currently the update only happens if a comment is found with matching signature (which includes the head commit ref).

The only time that ref will be the same is when re-running a check.

Every other push, new commit, force push after ammend etc will result in a new comment and many such comments.

I'd prefer one message per PR, if that cant be the default then perhaps an option for only one comment.
