xml-binding continuation scheme is bogus

There's KK, which represents the compile-time continuation, and K, which represents the runtime continuation. But the runtime continuation is never actually used so it should be ripped out. See xml.lisp in ZAWS for an example.

The XML binding is pretty bogus in general and should be reworked, but for now...
