Ensure the fake agent in integration tests doesn't swallow exceptions

This PR reverts #4991 and implements a better implementation as we now understand the problem:

The issue with the ignored exceptions only occurred when using the fake agent. The fake agent is an Express app and Express will by default catch any errors that happens in its route handlers and return a 500. This means that some of our tests didn't fail if the assertions threw inside of a route handler.

This PR works around that issue by adding a custom error handling middleware to the fake agent Express server that re-raises the caught error after the next tick.
