Small simple IP blocker for Connect/Express.

: Boolean - If true, treats passed IP(s) as whitelisted. Otherwise, treats them as blacklisted. Default is true. -
allowForwarded
: Boolean - If true, checks various forwarded-for headers for an IP. Default is false.
