DEVEX-1823 readahead and 255k dir limit (#58)

* Raise directory limit from 10k objects to 255k
* Change prefetch heuristic for plink access pattern
* Create remote files as read-only if enabled
* Skip posix processing if filenames are unique in dir with no subdirs
* Bump version to 0.23.0
