chore: dependency updates

This updates everything except cross-spawn, which has changes that cause test failures and reordering and is a non-trivial update.

I've updated tap-parser all the way to 10, and made some significant changes to the reference test output to accommodate its additional information.

dependency-check 4 requires node 10, but they fail open so the test suite can be run under node 8 by npx -p node@8 npm test to verify compatibility back that far.

These changes do eliminate support for node 6 though by updates to tap-parser bringing in a tap-yaml that does not work there.
