Remove ez_setup.py

This was previously the recommended way to install setuptools
when the user had a Python environment, but no setuptools.
For some time now, this has been a pretty rare case; further,
setuptools has deprecated this method
(pypa/setuptools#581)
and instead recommends the `get-pip.py` script, which will also
install pip.
