BREAKING-CHANGE: Removed deprecated label, design_parameters and document_version and replaced with required document.metadata field. Migrate documents to account for new metadata format.

Summary
I've added DCMI terms in a list format just now (quitevanilla). This is subject to change, we may use object form and may constrain some terms to not be listed values (eg to have single entries for identifier, type, etc).
