There was some missing formatting due to a minor mistake in the format check. That fix is rolled in here.
This PR adds license headers to all of the source code files. Hopefully the copyright lists are reasonably complete. They should cover significant contributors to a file, and the range of years they contributed. If you are missing from a file and you think you should be in it, or you want your credit to be modified some other way, feel free to send a PR or file an issue.
License headers may not always be necessary, but they are generally advised.
