sonic-mgmt: t0-isolated-d128u128s2 leaf template (#16022)

What is the motivation for this PR? Restore a leaf template file for t0-isolated-d128u128s2-leaf in 202405 that was unintentionally deleted in manual PR 15786. How did you do it? Restored the symlink file. How did you verify/test it? Verified the symlink points to the contents of the underlying t0-leaf.j2.
