Cycle completions

Support cycling through completions. Uses all the work of the last few PRs to make it happen.

• By default, I use precedent, and the Copilot VS Code plugin uses option-] to toggle between selections. But option-] is a real key combination for writing ‘ and I use it fairly often, so I am not going to use it as a default.
