Fixes to vkCmdPushDescriptorSetWithTemplateKHR().

- Fix crash when entries in VkDescriptorUpdateTemplateCreateInfo
  are not sorted by offset.
- MVKCmdPushDescriptorSetWithTemplate allocate new data memory
  only if existing memory allocation is too small.
- mvkCopy() support void pointers, and copy a single value directly.
