Can this be used to have an item-based currency?

Yes, you have to change the currency type from "virtual" to "item."

Can you share balances across worlds?

Yes, you can use the world sharing options in worlds.yml, or change Multiworld to false in config.yml if you wish to share with all worlds on the server.
